जीआरपी थाने में हुई युवक सुनील की मौत के मामले की जांच में कई अहम सुराग मिले हैं। इसके अलावा जीआरपी पुलिस ने अब इस मामले में सहयोग करने वाले सात सिपाहियों को भी लाइन अटैच कर दिया है।
...पूरे मामले की जांच भोपाल से आई जीआरपी की डीएसपी इमरिन शाह ने की। इस दौरान सबसे अहम सुराग वह बक्सा बना है, जिसमें सुनील का शव चंबल नदी तक ले जाया गया था। यह बक्सा लकड़ी का है और जीआरपी थाने में रखा हुआ मिला। इस बक्से में सुनील के शव को रखकर कीलों से बंद किया गया था। इसी के चलते यह बक्सा टूट गया। यह टूटा बक्सा रिकार्ड में लिया गया है। सूत्रों के मुताबिक जीआरपी के सिपाही महेन्द्र सिंह बुंदेला ने ही बक्से में कीलें ठोंकी थीं। बुंदेला के हाथ में इस प्रकार के निशान फोरेसिंक जांच में मिले हैं। इसके अलावा जेल में बंद दो कैदियों, दिनेश गुप्ता व पप्पू राठौर सुनील के साथ मारपीट करने की पुष्टि पहले ही कर चुके हैं। इन्हीं सब के चलते जीआरपी के आईजी आरके गर्ग ने राजधानी पहुंचते ही ग्वालियर स्टेशन के जीआरपी थाने में पदस्थ सात सिपाहियों, महेन्द्र सिंह बुंदेला, राकेश शर्मा, रवि सेन, दिवाकर, मनोज, धर्मेन्द्र व योगेश को लाइन अटैच कर दिया है। इस मामले में पहले ही इंस्पेक्टर आरएस चौहान को लाइन में भेजा चुका है। सूत्रों के मुताबिक एक दिन पहले पूरे मामले की एफआईआर दर्ज करने की तैयारी हो गयी थी, लेकिन चौहान ने अपने राजनीतिक संपर्को का इस्तेमाल किया और एफआईआर को रुकवा दिया।

Sir this is a serious case of human rights violation. On behalf of PVCHR, I request you to ask the state government for instituting a judicial inquiry under Crpc 176(1A). NHRC should also find out whether the videography of the postmortem and the report has been submitted or not. PVCHR demands that the victims family must be provided with financial compensation immediately.

Owing to the gross negligence of a doctor, two nurses and a compounder, a woman died after delivery in Dabhaura civic hospital of Rewa district on Wednesday.

Later, the Block Medical Officer (BMO), who visited the hospital, found it was sheer negligence of the hospital staff, following which the woman died.

Surprisingly, not a single pre-labour test was conducted by the hospital staff and the villagers have alleged that the staff are discriminating against the SC/ST patients of the region.



More surprisingly, the operation of the victim, Parvati Sen was done in the absence of the doctor posted in the health centre.


"She was operated upon at about 11 pm, but the nurses had referred her for Rewa at 2 am (after midnight) when she was almost dead due to excessive bleeding. The Janani Suraksha van was parked but the driver was missing. When we urged to compounder Ramesh Kumar Tiwari to help, he said that it was not included in his duties. Probably, this treatment towards us was because we belong to lower caste", said Dashrath, uncle of the victim.

Block Medical Officer (BMO) NK Pande also visited the civic hospital of Dhabhaura on Wednesday morning with the local police. Talking to The Pioneer, Pandey said, "There was negligence on the part of the nurses and the compounder and I have visited the hospital and took the details of the case."

He further said that he would submit a report to the CMHO and proper action will be taken after the investigation.

On the other hand, the villagers of Dhabhaura region have claimed that the staff of the hospital are discriminating against them on the basis of case. "We were rudely treated by the hospital staff and they often did not provide us medicines free of cost available in the hospital," a villager, who belongs to SC/ST community rued, pleading anonymity.


TI Dabhaura BS Parihar also accepted that the victim could have survived if the van driver would have turned up on time. "It's a matter related to Heath Department, but it is a fact that there was some negligence on the part of nurses and compounder," he added.


CMHO Dr BN Sharma said that he would look after the case and if there was any negligence, appropriate actions would be taken.

Meanwhile, the residents of the village and the relatives of the victim have alleged that police and Heath Department are trying to hush-up the case owing to the pressure of some affluent of the region.

 

Therefore it is kind request please take appropriate action at the earliest.

    हम आपका ध्यान झारखण्ड के लोहरदगा स्थित हंटरगंज के मुर्की तोडार पंचायत के ठकरू उराव (साल- 60 ) व उनकी पत्नी तेतरी उराव (साल-55) तथा जबडा के चिरैयाटाड. के जीतन मान्झी व उनकी पत्नी कपूरवा देवी की हत्या डायन बताकर की गयी, इस वर्ष लगभग 150 मामले इस तरह के सम्बन्धित राज्य मे घट चुका है, पुलिस थाने मे 1200 मामले दर्ज भी है, फिर भी नर बलि रुकने का नाम नही ले रही है, इस कुप्रथा से राज्य के कई जिले प्रभावित है!  
   अत: महोदय से निवेदन है कि उक्त मामले मे कार्यवाही करते हुये दिशा निर्देश जारी करने की कृपा करे!

विषय:- बाढ़ से प्रभावित लोगो तक राहत एवम पुनर्वास कार्य नही पहुचने के सम्बंध में।

महोदय,

लेख है कि वाराणसी उत्तर प्रदेश में विगत तीन दिनों में गंगा एवं वरुना नदी के जलस्तर में तीव्र बढ़ोत्तरी से वाराणसी के तटीय इलाको पर स्थिति मुहल्ले पानी से डुब गये है। मध्यरात में घरों में अचानक पानी आने की वजह से लोग अपने दैनिक प्रयोग की अधिकतर वस्तुओं को नही बचा पाये। अभी लोग मस्जिद्, मंदिर व खाली स्थानों पर अपने सिमित संसाधनों से रुके हुए है। वाराणसी प्रशासन के द्वारा राहत कार्यो में शिथिलता बरती जा रही है। राहत कार्य के नाम पर कागजी कार्यवाही अधिक हो रही है। लोगों के पास राशन, दवा, पानी व अन्य आवश्यक वस्तुओं की आपुर्ति लगभग नही हो रही है। जहाँ जलस्तर कम हो रहा वहाँ बिमारी फैलाने की आशंका है। जबकि दवाओं का छिडकाव नही हो रही है। लोगों के आजीविका का साधन नष्ट हो गये है।

                महोदय, इस सम्बंध में निवेदन/मांग है कि बाढ़ से प्रभावित लोगो के पास राहत आपुर्ति पहुँचने,पुर्नवास एवम मुआवजा हेतु आवश्यक कार्यवाही करें। महोदय बाढ़ से पीडि़त लोगों में अधिकतर बुनकर एवं मजदूर परिवार है। अतः सरकार द्वारा की जाने वाली बिजली एवं अन्य ऋणों की वसुली पर आजीविका के कार्य शुरु न हो होने तक रोक लगायी जाय।

महोदय,

वाराणसी जिले में गंगा एवं वरुणा नदी के आस- पास के इलाको में रहने वाले परिवारों को इस समय बढे हुए पानी के करब बहुत मुश्किलों का सामना करना पद रहा है. नक्खी घाट, शैलपुत्री, मीरा सिधवा घाट, कोनिया आदि सभी क्षेत्र मुस्लिम बुनकर बाहुल्य क्षेत्र है जहाँ बुनकरों में भी आर्थिक रूप से अत्यंत कजोर तबके के परिवार निवास करते हैं.
बाढ़ के प्रकोप से परिवारों को इस समय अत्यंत विषम परिस्थितियों का सामना करना पड़ रहा है.
अतः बाढ़ प्रभावित परिवारों को अविलम्ब जिला प्रशासन की तरफ से आपात कालीन राहत प्रदान किये जाने का निर्देश दिया जाय.

Launched in 1975, the Integrated Child Development Services (ICDS) has covered 7.6 million expectant and nursing mothers and over 36 million children in less than six years. It would not have been possible without the services of anganwari workers and helpers, who have a target to achieve six goals of child health and nutrition.

But even after all these years, the condition of anganwari workers and helpers remains pitiful. Most states do not compensate them for their services let alone improve the infrastructure at anganwari centres. As on July 8, 2011, over 60 per cent anganwaris did not have safe drinking water sources and were depending on non-treated water while only 40 per cent had toilets.

Although a major duty of the worker is preparation of meals for children daily, a whopping 75 per cent anganwari centres do not have kitchen facilities and cooked food is often left exposed to hazardous materials. Only 17 per cent anganwaris were found to have clean kitchens. Haryana had the highest number - 84 per cent - unclean centres; while Punjab had the dirtiest toilets, not one surveyed toilet being clean.

As if that was not enough, as many as 11 states do not consider an anganwari worker and a helper's worth more than zero to Rs 400 a month! They make no additional payments to these workers, who must manage with the salary the Centre gives. Since ICDS is a centrally sponsored scheme implemented through the states, bulk monthly salaries come from the Centre, which recently revised the anganwari worker's monthly salary from Rs 1,500 to Rs 3,000 and a helper's salary from Rs 750 to Rs 1,500. Every state was expected to top this up with Rs 1,000 for a worker and Rs 750 for a helper, taking the overall salary to Rs 4,000 and Rs 2,000, respectively.

But Nagaland, Arunachal Pradesh, Mizoram and Bihar pay nothing additional to workers. Meghalaya, Manipur, Uttar Pradesh, Himachal Pradesh and Rajasthan pay Rs 100- 300 a month. Helpers are worse off, with Meghalaya, Nagaland, Arunachal, Bihar, Mizoram paying them nothing, Manipur paying Rs 50; Uttar Pradesh, Himachal Pradesh and Rajasthan paying Rs 100, Rs 150 and Rs 200 a month respectively.

Only Goa, Haryana, Tripura and Delhi are making respectable monthly contributions to anganwari workers giving them Rs 4,200, Rs 1,563, Rs 1,568 and Rs 1,000, respectively.

Since anganwari workers and helpers are important to the success of ICDS, the Planning Commission's review states: "Anganwari workers in India are overburdened, underpaid and mostly unskilled. Although their job is about six hours a day, they are asked to perform works of other agencies with or without incentives. An assessment of their time disposition should be done to see if they can carry out anganwari centre-related responsibilities." 

 

Sir, this report shows the inhuman condition in which the Anganwadi workers and others crucial for the welfare of the mother and child care are working. This sub standard working condition is a gross violation of human rights. This is a gross violation of human right. On behalf of PVCHR, I request NHRC to take cognizance of the case and direct for necessary action.

Besides supplementary nutrition, the Integrated Child Development Services (ICDS) scheme is also supposed to ensure healthcare delivery to children up to the age of six years, pregnant women and adolescent girls. Regular health checks, maintenance of registers pertaining to child growth and immunisation, and treatment of infections is integral to the duty of an anganwari worker. For this to happen, fresh medicine stocks are quintessential.

But the Planning Commission's evaluation report reveals that the medicine stock and child weight and growth registers are being poorly maintained across the country with only 33.7 per cent of the surveyed 1,500 centres in 35 states updating their registers regularly.

Also, 13 per cent of anganwari centres (AWCs) were found to have expired medical kits while 37.6 per cent did not have any medical kit at all for 2008-2009, the period of study. AWCs in Punjab, Chhattisgarh and J&K recorded zero receipt of kits. Also, the percentage of AWCs with expired medical kits was the highest in Punjab (50), followed by Tamil Nadu and Jharkhand at 40.9 and 28.2 per cent, respectively.

Surprisingly, only 581 big and 479 small tablets of iron/folic acid (anti-anaemia) were received at the surveyed 1,500 centres across the country. On an average, only three bottles of Vitamin A; 401 tablets of paracetamol; two bottles of paracetamol syrup, 94 tablets of Metrogyl (for diarrhoea) and 357 tablets of Mebadazol (anti-bacterial infections) were received by these AWCs.

Also, Punjab, J&K and Chhattisgarh received zero stocks of Vitamin A bottles; iron/folic acid; Metrogyl and Vitamin B complex. Punjab didn't receive any packet of ORS while Haryana received most medicines except paracetamol syrup of which it had nil stock. Less than 5 per cent AWCs received Vitamin A stock and less than 10 per cent received any eye ointments, with Punjab, Bihar, Chhattisgarh, J&K and Jharkhand receiving none. These states also reported zero receipt of Betnovate tubes.

Inability of the anganwari workers to update critical medicine stock registers led to nil or expired medicine stocks. Since no demand was made, no supplies came. The investigators found that most anganwari workers were not trained to update medicine, child growth and weight registers.

Nationally, only 42.3 per cent anganwaris updated child weight and growth registers and less than 50 pc updated immunisation data and survey registers, which list the eligible beneficiaries. Percentage of the centres updating daily attendance; birth and death registers was 84.9 and 95.6, respectively.

"The anganwari workers required to keep stock of survey registers and children's weight and growth charts, had little knowledge of the job. The attendance, birth and death registers were better maintained. Rest of the registers were not even traceable at most centres," the report concludes.

Region ignored

  • Over 90% AWCs in Kerala, Haryana, Jharkhand, Bihar, Andhra Pradesh utilised big iron and folic acid tablets while Punjab, J&K, Chhattisgarh didn't receive this stock
  • 87% centres nationally used ORS; Punjab, J&K, Chhattisgarh did not receive stocks
  • 90% centres reported the use of Metrogyl to fight diarrhoea; less than 10% centres received eye ointments and Dettol.
  • Except Tamil Nadu, no state had more than 60% centres updating medicine stock registers
  • No centre in Punjab, J&K and Chhattisgarh updated medicine registers

Sir, this report shows total failure in programme implementation of important programmes like mother and child care. The report proves that the beneficieries are deliberately deprived of their mandated provisions. This gross human rights violation should be investigated.  On behalf of NHRC, I request NHRC to take cognizance of the case and direct for necessary action.

Hailed as the world's largest child health programme that seeks to reduce severe to moderate malnutrition among India's 6 to 72-month-olds, the Integrated Child Development Services (ICDS) is showing deep signs of weakening.

In evidence of massive divergence of funds and low coverage of the eligible beneficiaries under the 36-year-old programme, the latest Planning Commission review has found out that across India, only 40 per cent of the expenditure which the state governments reported on the supplementary nutrition (SN) of children under the ICDS could actually be accounted for at the national level.

The rest of the expenditure could not be justified on SN as claimed by the states. For the financial year 2008-2009, the year of study, close to Rs 3,000 crore worth of funds allocated by the government for the Supplementary Nutrition Programme (SNP), the most critical component of the ICDS, were siphoned off. This amounts to 56.4 per cent of Rs 5,383 crore, the total budgetary allocation for the SNP component under the ICDS for the said year.

So far as coverage goes, only 41 per cent of the children in the eligible group

across India were registered at Anganwari Centres (AWCs); 38 per cent of the eligible pregnant and lactating women and 10 per cent of the eligible adolescent girls were registered. Of those registered, only 64 per cent of the children were receiving supplementary nutrition on an average for just 16 days a month, which works to 192 days a year as against 300 days of supplementary nutrition mandated by the Supreme Court under the ICDS.

Field surveys of 19,500 households being served by 1,500 Anganwari Centres running 300 ICDS projects in 100 districts across India showed that large proportions of funds meant for SNP were lying unused at several locations. Based on the evidence gathered from beneficiaries, the review found that the massive unspent funds meant for child nutrition were actually siphoned off.

The percentage of the unspent amount under the SNP component is the highest for Assam at 95 per cent, followed by Bihar and Rajasthan at 71 each; Uttarakhand at 72; Madhya Pradesh at 64 and Punjab at 56 per cent.

The review report states, "The divergence between reported expenditure on SNP and spending that could actually be justified on ground is a matter of serious concern. Collateral evidence suggests that in most states, a large percentage of unspent funds meant for SNP were siphoned off."

The review details the manner in which funds are being misappropriated. "In some areas where AWCs are given cash every month, there is a nexus between the Child Development Project Officer, the Supervisor of the AWC, bank, panchayat and Anganwari Workers (AWWs) to siphon off cash.

"In some others, contractors are supplying food items and money gets diverted through the manipulation of accounts and entries in the registers of beneficiaries the AWWs are supposed to maintain. There are severe irregularities in AWC registers and the ICDS lacks effective monitoring mechanisms," the survey concludes.

Sir, this is a gross violation of child right. It reflects immense non seriousness in the part of the state in implementing programmes for child development and about the way it treats its future generation.  On behalf of PVCHR, I request NHRC to take cognizance of the case and direct for necessary action.

Several anganwari centres (AWCs) across the country are perfecting the art of recording children's weight without even using a weighing scale while several others that have scales are entering wrong measurements into children's growth registers.

Nationally, over one-third of the 1,500 anganwaris surveyed in 2008 and 2009 didn't have functional weighing scales for babies; more than half (60 per cent) didn't have scales for adults - pregnant women and adolescent girls. The lowest availability was reported by the anganwaris of Punjab, Jammu and Kashmir, Bihar and Rajasthan.

The new evidence has brought under question the veracity of children's nutrition progress data being produced under the Integrated Child Development Services (ICDS), with the Planning Commission's first major review of goal delivery under the scheme, stating, "Child growth and nutrition data being generated by the states and the Ministry of Women and Child Development (WCD) doesn't represent the grassroots reality. This mismatch is a major source of leakage of resources under the ICDS."

The scheme has failed to effectively deliver services critical to reducing high infant, child and maternal mortality rates (IMR, CMR and MMR). These services are - regular weight monitoring of beneficiaries, provision of nutrition, health education (NHE) and immunisation to beneficiaries, encouraging mothers to seek medical help if their children fall sick and de-worming children aged 0 to 6 years.

Investigators interviewed beneficiaries from 19,500 households serviced by 300 ICDS projects in 1,500 AWCs across India and found that the scheme had some positive impact on normal children and those suffering moderate Grade-I malnutrition, but failed to improve the nutritional status of children with severe Grade II to IV malnutrition.

Provision of supplementary nutrition to beneficiaries remains poor - 16 days a month for 0 to 6 year olds; 12 days a month for pregnant women and six days a month for adolescent girls. Under the ICDS, food must be given 25 days a month. Even the WCD Ministry admits, "The states are not meeting nutrition norms." Assam gives supplementary nutrition for just two days a month and Uttarakhand for eight days.

Weighing of children at birth is critical to reduce the IMR, but nationally the impact of the ICDS is marginally negative on this front implying that it hasn't made progress in getting children weighed at birth. "The practice is significantly absent in the beneficiaries of Punjab, Himachal, West Bengal and Karnataka. Jharkhand is good," the report says. Overall, only 46.5 per cent children were weighed at birth and 43.2 per cent were subsequently weighed every month.

The report concludes, "Most services critical to attacking the IMR, CMR and MMR in both short and long runs have not been effectively delivered. Rather, services like pre-school education (PSE) that plays a subsidiary role have been better delivered. Service delivery for adolescent girls is the poorest."
